How do you serve a an attorney that uses a po box as an office address- no other address available?

Your local sheriff, marshal, or constable will normally get the job done.
Their prices are more than fair for the work and stress they save you.

You can always be smart and hire yourself a process server.
they earn their money, trust me.

You can loiter around the courthouse, and he's sure to show up sooner or later.
You can serve him at or in the vicinity of his home.
A smart process server could have someone pose as a client and arrange a meeting.
That clever process server then swoops in when the lawyer appears and hands him the GOOD NEWS.

If permitted in your state, you could serve via registered or certified US MAIL, return receipt requested (FedEx, USPS, and UPS overnight letters, with proof of delivery are also acceptable).

You could contact the state bar and see if they can offer you additional information about the whereabouts your missing, Perry Mason.

In some cases, service by publication might be acceptable.
Check your state's laws.

Oh, yeah, ask other attorneys.
In many cases, they know how to contact each other.
