Shoplifting, Larceny, Robbery, Theft Bad Address Civil Demand

Okay, I did a dumb thing by taking some items totalling less than 50 from a store, and I feel horrible about it. Got caught, was really shaken. Because I gave the item back the police were not called. I had to go inside to sign a few things, give some information. I did a second dumb thing because I was very shaken. I gave all bad information, changing little details like shifting numbers around and not using my first name, wrong address, wrong street address, didn't give my social security. Any idea what's could happen? I have a ban, and a paper that says to expect a civil demand.
 
Forgot to mention that they didn't look at my ID, and never walked around the back of my car [I got to the parking lot] to check my plates.
 
For anything special or just the theft? I didn't sign anything that had me agreeing to pay, they just said they'd send that, what I did sign was a paper saying I'm no longer allowed in the store. Also do you have any idea the likelihood of them going very far to find me? Thank you much, it's appreciated that I've found someplace to ask these questions.
 
Not to mention, with the wrong address, you won't get the civil demand, and the cost will keep escalating since you won't know to pay it and could cost you thousands. I"d call the store and give the correct info and save yourself some pain, and possible criminal charges.
