Breaking my lease

I am currently living in an apartment with a 12 month lease. During my time here I have had numerous problems. Our refrigerator leaks, the stove/oven is old and broken, mold is growing around our tub, the hot water doesn't work, we have no heat (the a/c unit broke in July but instead of fixing it they gave us a window unit), and there is a terrible roach problem. All of these issues have been presented to the rental office yet we see no results. A while back I sent an email to the management company because of other issues (the ceiling fell and left a hole for 3 weeks, kids outside busted our window and stayed broke for 2 weeks) and they were fixed within a day. I tried the same tactic again but nothing was done. Ironically other tenants at the complex are having the same issues and have posted signs persuading others to call the management company, but I just want out. I am currently looking for a new place to stay. I want to know if I would be able to break my lease without any penalties. If so, what would need to be done and what steps should I be taking now?
 
Where I see grounds for breach you must follow your unamed states regulations to resolve issues with documentation. I woud suggest you talk to a Landlord/Tenat Attorney. I would also suggest a call to Health Department
