You may want to ask the clerk of the court as to the appropriate procedure to use to correct an error in an order. A nunc pro tunc is typically used to retroactively make a change when something was unknown at the time the original order was issued or to make a remedy to prevent damages to a party as a result of a clerical error. I'm not sure if a standard motion might suffice in this instance but, if you want to prepare a nunc pro tunc order, the court may provide the proper form. Here is one that generally follows the form I've seen in other states.

Sample Nunc Pro Tunc Order Form

This one is for the Superior Court in Louisiana, which is a very different legal system. However, most of these are the same since the matter is simple and the form provides the necessary information for the court to consider.
