Cremation

I am in the process of drawing up a living will and have been told that there is a special Indiana State form that needs to be included regarding my request for cremation. Can anyone tell me if such a form exists? If so, can anyone provide the name and form number? None of the forms services seem to offer such a form, and I had thought it would be taken care of in the Living Will.

Any direction will be appreciated.

A living will has nothing to do with cremation. It is a power of attorney that dies when you do.

I suggest you hire a lawyer to make sure everything is done right because you are very confused.
