Dance Waiver?

Hello,

I am going to be hosting a dance to help raise money for my Relay for Life team, and I was curious to see if I needed to have a waiver. I have drafted up a waiver, but is it a strong enough one? Also, will it work if I have parents fill it out electronically?

I understand that participation in the LVHS Spring Benefit Dance could include actions or tasks which might be hazardous to the participant named below.
By signing below, I assume any risk of harm or injury which might occur to the participant due to their participation in the event or activity. I release LVHS Students for a Cure, the Carver Center, and Loudoun County Parks and Recreation from all liability, costs and damages which might arise from participation in the above named event or activity.
If the participant is a minor, I agree that the minor has my consent to participate in the event. I further provide my consent for LVHS Students for a Cure, the Carver Center and Loudoun County Parks and Recreation to seek emergency treatment for the minor if necessary. I agree to accept financial responsibility for the costs related to this emergency treatment.

Did the facility where you're holding your event require you to have insurance?

If it didn't, you might want to consider investing in some for the night!
