Doctors Note / Medical Malpractice??

I was resigned at work to a position I can not handle due to a disability. I went to the doctors, was seen by an M.D. My regular doctor was out of town so I seen an affiliate. The office manager wrote the note, normal practice there.. signed the doctors name and then initialed it with her own. I turned the note in to HR and asked for my rights under the ADA. (been with the company 21 years) HR told me not to return to work until they could find a position for me, and then said they wanted a more detailed note. I had to wait until my doctor returned to get a new note. Now my doctors office is saying the note is forged because it wasn't signed by the main doctor and refuse to write a new one, in fact they hang up on me if I try to call, I have been off work 5 weeks now and am destitute and only a few days away from being terminated. I have copies of everything.

I suggest you speak to a criminal defense attorney.
People get indicted for forging medical notes.
Its very serious.
I'm not saying you did anything wrong, but if they think you did something wrong, its time to lawyer up.
 
It isn't sounding like you are in trouble of a legal sort but if you can not get a note from your doctor, I am not sure what you expect your employer to do. They certainly can not get a valid note or details about your medical condition from a doctor who will not even communicate with their own patient. I highly suggest finding another medical practice.
