We are trying to domesticate a judgement to Okaloosa county, Florida. We were told in order to do this we need to complete a Affidavit of Last Known Address and file it. How can we obtain this form and is it specific for each state or for each county? We have found some sample Affidavits and Certificates of Last Known Address" online, but for states other than Florida. We are wondering if we can just create our own form based on the samples we are seeing online. We would be happy to pay for one online, but we aren't even sure we are downloading the right one. We don't want to pay for a lawyer just to get this simple form.
