employer doesn't issue a paycheck

Hi,
I completed a one day assignment as a temp for the travel medical personnel company. I was told that I would be issued a check a week after the assignment would be completed.
Yesterday, I was contacted by the company's human resources personnel who stated that I have to fill in the employee application, provide them with the CPR card and sign a contract. She also stated that I wouldn't get the check unless I do that.
Before I completed the assignment, the human resourses did the background check and received copies of my driver's license and the professional license. Why do they need all this additional info if I already completed the assignment and may never do another one with them again?
Plus, I don't feel comfortable signing any contracts after the assignment was already completed. I don't think I should fill in the employee application if I completed an assignment that was just one day long and I may never do the similar assignment with this company at all.
Can they not pay me?
What should I do? It has been three weeks already. Originally I was promised to get the check within 7-10 days.

Contact your state's department of labor and file a complaint of unpaid wages.
