Fact Info Sheet/Small Claims - do we have to send it to litigious Plaintiffs?

My jurisdiction is: Florida USA

We own a small condo as a second home in a 12-unit condo complex on the beach. The lady who owns the condo below us filed a claim against us in small claims court saying our bathroom leaked and caused damage to her bathroom ceiling. She has a hostile relationship with us and the other condo owners. To say that she has mental-health problems is putting it mildly. She would not let us or our plumber inspect her bathroom ceiling. Because of her prior history of lying and suing the condo assoc., we were relieved when she later said that she would handle the alleged repairs through her insurance company. A year or so later had us served and filed against us in small claims court. The judge unbelievably accepted her testimony and ruled against us for $800 (court costs and the alleged cost of the repairs.)

She asked the judge to have us fill out a Fact Information Sheet - Individual, Small Claims Rules Form 7.343. We had no idea what that was and were shocked to see the personal information it asks us to provide to her. Can we appeal the filing of this sheet? The Plaintiff is a litigious, angry woman who has threatened to sue us until the cows come home. She is also a practicing lawyer in NYC. We don't want her to know what our assets are.

Any advice would be appreciated. The suit was filed in Brevard County, Florida. Thanks.
 
Filling out the form is something that can be required to assist the successful party in collecting their judgment against you. If you simply pay the judgment, filling out the form should be unnecessary. But I would check with the clerk of the court to be sure. You don't want to be found in contempt.
