Form to apply for permission to take child to UK

In the UK the form is called a 'Leave to remove' form to apply to the courts for permission to take a child to another country. My child was found to have jurisdiction here in the US and I want to apply to the court in Maryland for permission to take the child back to the UK. Does anyone know the name of the form for me to apply?

You need to enter a motion to do so. Is the child a US citizen or a UK citizen? Generally, a US court does not exercise jurisdiction over a foreign child and does not usually allow a US child to leave.
