forms for full custody

Okay I am a handicaped mother of an almost 2yr old. her father has a agreed to give me full custody all he wants is vistation. I am wondering what form will I need and how to go about this.

Has dad established paternity?
Has dad acknowledged paternity?
Is dad paying child support?
Who is responsible for paying the child's health care?
What happens to baby, IF something happens to you?
What happens to baby, IF you get ill?

Dad wants visitation, sure.
Has dad agreed to pay child support?
If not, you should discuss that issue, too.

You should speak to a lawyer.
A child isn't property.
There are many more things to consider and protect (than just custody).

If you can't afford an attorney, the state will help you.

Who is responsible for paying the child's health care? dshs covers her health care my ex has no health coverage or any medical from his work and he does not pay a cent of her health care. but when shes sick i'm the one paying for her meds and such.

What happens to baby, IF something happens to you? I am working on a will that she will go to her godmother if anything happens to me her father admits hes unable to raise her.

What happens to baby, IF you get ill? right now i'm healthy but again godmother will get her.

Dad wants visitation, sure. yes he only wants vistation, he said no joint no custody just vistation.

Has dad agreed to pay child support? in a way he has but its still getting taken out of his checks. and not writing checks I get TANF

the reason why I want custody is half the time I cant get a hold of him second hes a acholoic even though he is an adult he still has alot of growing up to do. I wants whats best for her.
