How do I evict a housemate?

I am one of 5 college women in a house in NJ. We have all signed the lease over a year ago, agreeing to many stipulations including that we share the cost of utilities, rent, repairs etc. 5 ways. One housemate is consistently paying late and not cleaning up after herself, rotting food, dishes in the sink for weeks (no kidding) and general ignorance for other members of the house. I've spoken to my landlord and showed him an Instant Message conversation of hers/mine detailing how her she said, "my name isn't on the bills so what do I care?" which leads us all to believe she is going to screw us on the UTL in the next months (we have a whole year left). So, the landlord said that we, 4 other girls, have to write up a notice and give her 30 days from the date to move out. MY QUESTION IS - at that point, 30 days, what can I do if she is NOT gone? What do I do if she doesn't pay her 1/5th of everything? DO i need this document notarized? Do i need the landlord to sign it as well? YOUR HELP IS SOOOO APPRECIATED!
 
This is always a problem when you don't have an agreement. If you've only got a month to month agreement with her - which is what it seems to be - then a 30 day notice of termination means you can start minimizing your potential losses. Good luck to you.
