How do people file for adult guardianship?

My dad was in an accident and has TBI. I've been trying for months to get a doctor to cooperate with getting the Physician's report filled out that I was told is required to even get a lawyer to be able to assist with filing for emergency guardianship. While my dad is currently in a nursing facility in Indiana for the foreseeable future, and he is unable to function on his own, I am stuck being unable to take care of his finances or other business. Since the accident, he has been in hospitals and the nursing facility and essentially has no real PCP that can assess him and fill out the paperwork. The nursing facility says they only have a doctor that comes around once a month to check in on patients and would not be able/willing to fill out this paperwork. None of the hospitals have been able to assist and have told me to get someone from the nursing facility to fill out the report. I don't know who to turn to to make this happen.

Told by whom? I doubt very much that a competent attorney who practices in this area will require that you have some sort of "physician's report" before taking the case.
