Lay Off In California

I am an employer in California preparing to layoff six employees in a few weeks. One of them is my HR Manager who has no idea a layoff is coming. We are laying off b/c of the the economic downturn.

I have never processed layoff paperwork before and therefore an in the dark as to what I need to provide each individual. What I suspect I need but need validation is:
1 - Letter written to employee indicating their position is being eliminated
2 - Final check with vacation payout and severance.
3 - COBRA info but do not know where to get this
4 - Employment Development Department Handbook

Do I need anything else?
Do I need to notify the state that an employee has been terminated?
How long do they stay on our benefits plan?

Any help you can provide is appreciated.
