My mother is trying to terminate my rights?

My mother has guardianship over my daughter. She refuses to let me see or talk to her, I don't know where she lives and my daughter doesn't even know I exist. I have tried to get the court ordered visits that I am allowed but I don't know how and after I filed a petition to get my visits, my mother's attorney served me in the courtroom with adoption papers. They say that I abandoned my daughter and on that basis they want to terminate my rights. I never would do that, and because my mom wouldn't allow me to have contact I might lose my daughter. I also have full custody of my two sons, her brothers that she has never met. I need help PLEASE, anyone. Thank You.
