Need advice on my rights

My wife found out recently that a complaint was filed against her by a nurse or nurses in a unit of the hospital that our grandson is at...it's an intensive care neonatal unit....... The patient relations person that notified her about the complaint (for abusive or threatening phrases and other things which are either false or only half true) said he did not have to give her a copy of the complaint since it was just notes that he had wrote down. We later found out that a hospital-wide security alert had been sent out to all departments stating in part that (my wife) was believed to have mental problems and showed erratic visitaion times and had also made threatening comments to the nursing staff.

My questions are:

a) Wouldn't it have to be a formal complaint to lead to all the security alerts..

b) Don't we have a right to get a copy of the complaint in writing whether it is a formal complaint or just notes that are wrote down?

c) Would this be a form of slander? What would we need to do to prove that it is in case the situation gets any worse... I have already told her to try to get a meeting with the hospital administrator and director of nursing and whoever else needs to be there also.
 
There is no "we" here. You are not a party to your wife's employment.

No, they are not obligated to give her a copy of the complaint regardless of the form it takes. You have no standing to receive anything at all.

Based SOLELY on what you have posted, no, it is not. She (not you) would have to prove that the nurse knew for a fact that what was being stated was untrue. An opinion is not actionable.
