playing attorney need to know what form to use?

I am in a limited civil suit with my ex mother in law. I need to know what a verdict statement is and how to go about doing one? And I also need to know what form it is to use to put my jury instructions on?

okay I figured out what the verdict statement is, but now I need some advice on a closing statement......anything that may help please.
