Property taken away by state?

We live in PA and the property in question is in WV.

My uncle used to own some property in WV. A little over two years ago he sold it to my father. About a year ago my uncle went into a nursing home. He is no longer able to work and is a ward of the state. When they put him into the home the people at the home messed up his paperwork(they have been fired). But now the home/state i am not sure who is saying he owes them $7000. They said they can go back up to 5 years ago and take back his assets that he may have. In PA there is a law that says the state can do this because people have tired to hide assets before. But we had no clue my uncle was going to go into a home either. I guess my question is would the state have to proove that we tried to hide the land, even though we didn't, in order to take it? And how is my uncle liable for the $7000 if the nursing home employees were the one to mess up?
 
Who's the bill comming from? Is the $7000 directly due to the paper work mistake, or something else?

Was the paper work ever corrected?
 
I just found out the the bill is coming from the home that he is in. My dad got a letter from an attorney for them. And yes the $7000 is due because the paperwork did not get in on time. The two people who were doing the paperwork had a lot of other issues as well. The paperwork was corrected and now the state is taking care of everything but the home wants it's $7000 even though it is not my uncle's fault.
 
So, if I understand you, with the CORRECTED paper work, there would be no $7000 bill?

I would write that debt collecting shyster, send the letter certified, return receipt. Ask him to verify the debt, and to send you/or your father certified copies verifying the debt. Then, you could send him copies of the corrected paper work, and explain you don't owe anything.
