Question pertaining to Power of Attorney form

Hi, I'm not sure if this is the correct place to post this question but I am a young novice and would like to know how to properly fill out one section of a Power of Attorney form.

I have the Form Gen-58 from: Power of Attorney and Declaration of Representation

The question is how do I fill out the Tax Matters section? There are two boxes: Type of Tax and Year(s) or Period(s).

The tax I want listed is a State Tax Lien, from the year 2013.

I had submitted the Power of Attorney form filled out and it was returned with a letter stating it was filled out incorrectly. "Required data is missing, Please provide type of tax required under Section 3 tax matters."

So does anyone know how to go about properly filling out this section so the form isn't returned?

Well what are you trying to do and for whom or what? The form is pretty specific - it shouldn't be a mystery. We can't answer your question because only you know the answer!
 
I'm thinking now I can put Individual for the type of tax. As a Tax Lien is only the result of a tax requiring payment? And for year(s) or period(s) I will try all pertinent years and list the year the tax was accumulated and every year since. Instead of the Tax Lien filing date which come to think of it doesn't matter in this case.

Maybe this will work, I think this makes sense. Although it's still all greek to me...

Well what are you trying to do and why? Who is granting you PoA and why?

That is irrelevant to my question. I don't need legal advice, just confused as to how to fill out a form so it is accepted by the DOR. I think I have a handle on what to put down now so I will just have to see how it goes.
 
Well, we don't read minds here - and it could be very relevant.

Maybe you can actually read the form now and understand it. Why you didn't get it the first time is anyone's guess.
