Request for info on acct on credit report?

Does anyone have a form or letter to request information published on a credit report? I have something from "NCO FIN/38" for $16.00. I haven't recieved any bills/letters stating I owe $16 or any amount. It only lists an address, no phone number!? I googled the address and came up with AssetCare, Inc. and still can't find a phone number. I want to send a letter requesting more information on this account and why I haven't been contacted. Any ideas? I can come up with a basic letter, but don't know any legal talk! Thanks
