I have surgery scheduled on same day of court date.

Obtain a letter from the physician scheduled to perform your surgery.
Have the physician indicate the date of the surgery and how long you will be incapacitated recuperating from the surgery. The surgeon's letter should identify you with name, address, date of birth, as well as mention you are a party to a case on such and such a date in Judge Big Deal's court in Judgeville, State of Legalities.
The letter should be addressed to the judge with a copy to you and your lawyer (if you have one).
If you have a lawyer, ask him to address the rest for you when he gets the letter.

If you have no lawyer, mail (or hand deliver) a copy of the letter to the judge, provide an extra copy for the judge's clerk.

If you mail the letter, send it certified or registered mail, requesting return receipt (proof of delivery). Or, send the letter via overnight delivery, which will provide a proof of delivery.

Keep copies for your records. Follow up a few days later with the court clerk to make sure the judge gave you the required continuance.
