response to custody modification

My ex has filed a custody modification to our divorce of 2003. I don't know where to go to get the response form to stop this nonsense. I am now living in Texas and have been for the last eight years. The divorce took place in Illinois. I have been told to go online and find a response form and mail it to the county clerk in Illinois, but I don't know where to get the form.

You've lived in Texas for the last 8 years?
Yet, you were divorced 7 years ago.
Were you a Texas resident at the time of the divorce?

What did the divorce say about support, custody, residency, etc?

Who has custody of the child(ren)?
Where does the custody order require the child(ren) to reside?

The divorce was March of 2003. I have custody of the children in Texas. Our divorce decree states that I had permission to move the children closer to my family. i just need to respond back to this case management order. I have been told to file a response through the mail, to Illinois Circuit Clerk, with a self-addressed stamped envelope so they can send me back copies.
 
He is claiming that due to my job loss, I can no longer take care of the children. I have a job now and they are insured. He feels they don't socialize enough, yet they are in numerous activities and they are church leaders. I have custody, he gets visitation during the extended holidays, and summer.
