sworn affidavit issued by california court

I am recently widowed and currently in the process of obtaining my husbands retirement benefits.My deceased spouses' employer retirement admin is requesting that I provide a certified copy of our marriage license. That is no problem however they are also requesting a sworn affidavit issued by a Ca. Court stating under penalty of perjury that we were married from the time he started collecting retirement benefits ( June 2009 ) and his death (sept 2011) and that there is no valid decree of dissolution. It sounds to me like they want the courts to acknowledge the sworn statement (place my right hand on a bible??) via an affidavit. but when I contacted several courts in various counties not one of them in any of their many departments had any idea how I might go about executing such an affidavit....This retirement admin has been having me jump through hoops for the last 6 months and my finances are almost depleted so I am trying to avoid the legal fees of an attorney if I can so if any one has any ideas I would appreciate them....Thanks Glendle
 
You might have to talk to an attorney.

Did you ask retirement admin. where you can get one? (what court) If they requested them from others previously (seems they would have), you would think they would know where to get one & what they look like etc. I assume you told them courts don't have them that you contacted.

I wonder if it is something a lawyer might draw up for you?

We have at least one employer benefit's person on this forum - maybe they will come along & be able to tell you something more.
