Probably, but wages are attached for things other than child support.

Most of those request forms have contact numbers to call if you have questions.

I would just leave it blank. To me, that is not your responsibility to tell them and I think in at least some states, some garnishments are protected from retaliation or consideration (i.e. you can't refuse to hire because potential employee has a garnishment). If they want that info, they should run a credit check on said potential employee.

It isn't for purposes of hiring, it's something do with probation. If it were for hiring purposes it would already be in the trash.
