Fact Information Sheet

I have to submit a Fact Information Sheet for a judgement against me. There is a Spouse Related Portion. My wife was not a party to the action and does not appear in any form. Do I have to include her employment and payroll information? If not, I am also required to submit income tax statements for past two years which were filed jointly and contain mostly her income.
I am also asked to supply a Notice of Compliance to the Court. Is this a particular form or is a copy of the fact information sheet sufficient?
