If I submit Power of Attorney to an entity in another state, what then?

I have a form for the state I live in but need to submit the document to an entity in another state. Should I get a form (PoA form) for the state I am submitting it to or can I leave out the state format? It is for pension requirements. We live in Florida but the agency we are requesting Power of Attorney is in another state.

What is the scope of the authority conferred by the POA?

This is very oddly phrased. The "agency we are requesting Power of Attorney"? What does that mean? I'm also curious why, earlier in the post, you referred to an "entity," but now you've changed to an "agency."
